In wrinkles that form in the middle and lower parts of our face, volume loss and tissue looseness due to the effect of gravity are at the forefront. In addition to the factors mentioned above, the decrease in the fat tissue and fat pads in our face over time and the osteoporosis in our bones also have a great effect on the formation of volume loss. Fillers are widely used to complete the deficiencies due to tissue loss in this area and to eliminate and reduce wrinkles.
Fillers can be roughly classified into three groups as temporary, semi-permanent and permanent. After application, temporary products last for an average of 6-12 months, semi-permanent products for 1-2 years and permanent products for 2 years or more. In filler applications, temporary fillers are most frequently applied due to their safety and semi-permanent fillers are less frequently applied. Most of the products with longer lasting effects are used much less frequently due to the negative effects observed in patients. Hyaluronic Acid (HA) is the most frequently used of temporary filler products. HA is a substance that is normally found in our skin, provides structural support and nutrients to the skin and gives volume and fullness thanks to its water retention feature. Commercially produced HA has different forms depending on the area to be applied to the patient and for what purpose it will be used. There are also forms that contain anesthetics to cause less pain in patients during application. HA disappears after an average of 6-12 months after application. Since HA is a substance that is normally found in our body, allergic reactions develop very rarely and it has been used safely in filler applications for many years.
Among the semi-permanent products, the Calcium Hydroxy Apatite (KHA) filler, which has been used most frequently and safely for many years, especially in orthopedic clinics, is available. In these fillers, KHA crystals are in a gel. The gel part is absorbed in about 3 months and instead, collagen production is stimulated by the person’s own tissue cells. The feature of this filler is that it provides volume and structural support, especially in deep tissues, and is generally used for this purpose. Its permanence is between 1-2 years and it is among the fillers that can be used safely. In addition, this filler can be used by mixing with anesthetic substances to reduce pain.

Things to Consider After Application
1- On the day of the treatment, do not massage the treatment area, do not rub that area (especially care should be taken when removing make-up), avoid lying face down and applying heat.
2- On the day of the treatment, avoid alcohol consumption, exercise, sauna and hot showers, which may cause facial redness.
3- After the treatment, redness and swelling of the skin in the application area is expected and is expected to pass within a few days. If a condition lasts longer than 3 days, please contact your doctor.
4- Your doctor will call you for a check-up in 1-2 weeks and during this check-up, additional doses may be required. We can divide the unwanted situations after the filler application into two as the situations related to the injection and the wrinkle treatment itself.
Depending on the injection
Pain: Although very thin needles are usually used, there may be a slight pain.
Bruising and Edema (Swelling): Occurs due to subcutaneous bleeding in the injection area and usually passes within 1 week.
Redness: Usually occurs with every application and passes on its own in a few hours.
Depending on the Filling Material
Asymmetry: It may occur due to edema or dose inequality after application. The inequality caused by edema will pass on its own in a few days. Inequalities related to the applied dose can be balanced with additional doses in case of deficiencies and melting treatment in case of excess doses.
Allergy: Although very rare, an allergy to the filler material may occur. In this case, additional treatment methods are applied.
Subcutaneous Hardness and Masses
Depending on the sensitivity of the person to whom the application is made, fillers may rarely cause hardness under the tissue in the early or late period. In this case, additional treatment methods are applied.
Apart from these, serious conditions such as tissue loss and blindness that may develop due to the vascular-nerve anatomy in the application areas have been reported in the medical literature. Who Should Not Have Filling Application?
- Pregnant women, those suspected of pregnancy and those breastfeeding
- Those with active infection in the area to be treated (herpes, acne, soft tissue infection, etc.)
- In those with wound healing disorders
- In those with bleeding disorders
- Those with skin diseases in the treatment area (psoriasis, eczema, etc.)
- Those with uncontrolled systemic diseases (high blood pressure, diabetes, heart failure, etc.)
- People with unrealistic expectations or body image distortions
- In those with very severe allergies
